Discharge Plan
The Discharge Plan tile allows you to enter coordination and resident transition information.
💡 Some information entered in the Discharge Plan tile will pull over into the resident's Concurrent Review depending on the payer form selected.

Hours Needed per Day | Recommended care amount |
Notes | Freeform field for any notes |
Barriers | Freeform field for any barriers |
Resources Needed | Freeform field for any recommended resources after discharge |
Financial Concerns | Freeform field to document financial concerns |
Last Covered Day | The last day approved by payer |
Last Available Day | The last day offered for residence |
To edit information, select Edit from the ellipsis. Enter any relevant details, and then select Save.
