The HMO Log report provides managed care patient level changes per facility during the patient's stay. This spreadsheet includes admission & discharge information, insurance, and line of business information. You can also export this data to Excel.

Facility

Select which facilities or include All

Payer (Master)

Select “master” payer which includes linked contracts for that insurance

Select a month and year for report
